Home Visit M.D. assists patients from emergency/acute care to home care with our Independence at Home Program, Assessments, Integrated Transitional Program and Advanced Living Program. Our services range from a home visit to short-term care following hospital discharge to long-term care in a nursing home, rehabilitation center, assisted living facility and the patientís home.
